Title: The Function-guided Restorative Approach

Abstract:

In restorative dentistry, more is not always better, especially that it involves the sacrifice of healthy tooth tissues. However, less dentistry invites the risk of failure and may cause the sacrifice of the tooth as whole. Therefore, wise judgement is required to determine precisely where does enough dentistry lies.

On this lecture, a simple scoring system for the amount of functional loss will be proposed and linked with six levels of restorative interventions. Namely, stress dissipation cavity design, reliable bonding, glass fibres reinforcement, cusp capping, occlusal coverage and tooth crowning.

Introduction:

Three main factors determine the bio-mechanical stress response of the restorative system. The first is the magnitude, direction and type of load with special emphasis on cyclic lateral loading. The second is the cuspal strength measured by the ratio between the resistance arm represented by the base of the cusp and the force arm represented by the cavity depth. Lastly is the amount of structural discontinuity in the circumferential bio-rim encircling the occlusal table expressed by the number of auxiliary cavities emerging from the main preparation.
The possible reinforcing mechanisms include cavity design features that favor wide distribution of stress, thus avoiding the creation of stress razors and consequently surpassing the fracture resistance of the restorative system. Secondly, the utilization of bonding as a means of stress transfers from the weaker to the stronger parts of the restorative system. Thirdly, is by the use of high elastic modulus glass fibres as an intervening structure at the areas of discontinuities to provide the required support against cuspal bending and at the same time to decrease the net shrinkage stress thus lower the pre-stress condition on the adhesive bond. Fourthly, is the reduction of the cuspal height by tipping thus lowering the force arm with subsequent reduction of the bending moment on the cusp. Fifthly, it is by total occlusal coverage which re-direct the wedging force to compressive one that falls within the bio-mechanical tolerance of the loaded structures. Lastly, encircling the axial walls of the tooth to bind the various components of the restorative system and to counteract the hoop forces that emerge with severe mutilation. Encircling must not exceed the height of contour to allow for better resistance to the transferred stresses at the tension ring.
Treatment optimization requires a wise selection for the reinforcement techniques that correspond with the amount of damage fallen upon the restorative system.

About the Speaker:

Dr. Khaled Aly Nour graduated from Cairo University in 1990 where he joined the staff of the Operative Dentistry department. He obtained his PhD in 1999 then moved to Ain-Shams University where he initiated a gradual transformation towards adhesive dentistry. In 2005, as an Associate Professor he started contributing to postgraduate programs and by 2011 the department banned the use of amalgams after adopting esthetic and digital solutions with emphasis on preventive, holistic and minimally invasive dentistry.
Dr. Nour lectured and conducted numerous workshops all over the country as well as in regional countries as Libya, Kuwait, Oman, Qatar and Iraq. He holds one patency and has joined research with Minnesota and Harvard Universities and most of his published work is on minimally invasive dentistry and color science. He is currently a professor and the director of a number of graduate and postgraduate courses in the Department of Operative Dentistry, Ain-Shams University.

This 3-hour round table discussion offers a unique and interactive learning experience designed for oral healthcare professionals eager to hone their diagnostic skills. Attendees will be divided into three groups, each guided by an expert facilitator, to engage in solving challenging clinical cases, simulating real-world oral medicine mysteries.

Introduction:

Participants will have the opportunity to collaborate, share insights, and apply critical thinking to diagnose complex scenarios. Through dynamic group interactions and expert guidance, attendees will deepen their understanding of diagnostic processes, improve problem-solving skills via proposing management modalities.

About the Speakers:

Prof. Alexander Ross Kerr is a Clinical Professor in the Department of Oral and Maxillofacial Pathology, Radiology and Medicine at NYU College of Dentistry and in the Department of Otolaryngology at NYU Langone Health Center. Dr. Kerr is a recipient of NYU’s distinguished teaching award. He is a past president of the American Academy of Oral Medicine, director of the American Board of Oral Medicine, steering committee member of the World Workshop on Oral Medicine, member of the executive committee of the European Association of Oral Medicine, and member of the Scientific Advisory Board for the Oral Cancer Foundation

Dr. Alessandro Villa, DDS, PhD, MPH is a highly accomplished oral medicine specialist, researcher, and educator. He is currently the Chief of Oral Medicine, Oral Oncology, and Dentistry at Miami Cancer Institute, and a Professor at the Herbert Wertheim College of Medicine, Florida International University. Dr. Villa specializes in oral mucosal and salivary gland diseases, oral precancers, and oral complications from cancer treatment. He is also fluent in English and Italian.

Dr. Arwa Farag an Associate Professor at the Division of Oral Medicine, Department of Oral Diagnostic Sciences at King AbdulAziz University, and a Visiting Associate Professor at Tufts University School of Dental Medicine (TUSDM). Dr. Farag currently the Treasurer of the American Academy of Oral Medicine, a Steering Committee member of the World Workshop on Oral Medicine, a Member of the Scientific Council of the Saudi Board of Oral Medicine and Pathology, and an attending in the Oral Medicine section of the Oral Medicine and Pathology Post-graduate Residency Training Program.

Prof. Andres Pinto, DMD, MPH, FDSRCSEd, is Chairman and Professor in the Department of Oral and Maxillofacial Medicine and Diagnostic Sciences at Case Western Reserve, School of Dental Medicine. He has authored of over 70 peer-reviewed papers and abstracts, is a contributing author for three standard textbooks in the field of Oral Medicine, co-editor of one book on Risk Assessment for Dentistry, a reviewer for 12 scientific journals and an editorial board member of the Quad  O Journal. He is a past president of the American academy of Oral Medicine. Select honors include the American Dental Association’s Institute for Leadership in 2004, the 2005 ADEA’s Faculty Award and the Provost’s Faculty Award (University of Pennsylvania). Dr. Pinto is a diplomate of the American Board of Oral Medicine and holds a FDS from the Royal College of Surgeons (Edinburgh).

Demonstration

  1. Demonstration and Workshop:
  2. The workshop will demonstrate a step-by-step approach for direct composite diastema closure, focusing on predictable, aesthetic, and minimally invasive techniques. The session begins with case assessment and isolation, including evaluation of midline, tooth proportions, and rubber dam placement for moisture control. Next, emphasis will be given to shade selection and material choice, using composite shade guides and considering translucency to achieve harmony with adjacent teeth.
    The adhesive protocol will be illustrated through enamel etching, bonding, and controlled curing, ensuring optimal adhesion. The main part of the demonstration will highlight incremental composite build-up techniques, starting with palatal/mesial wall creation, followed by gradual addition of composite to close the diastema while preserving proper emergence profile and contact area. Anatomical contouring will be refined to achieve symmetry and natural tooth morphology.
    Finally, the session will cover finishing and polishing procedures, using burs, discs, and polishing systems to recreate texture, surface gloss, and natural luster. Each step will be explained in detail to equip participants with the knowledge and confidence to perform diastema closure in daily practice.

Dr. Yasser Alkayssi is a cosmetic dentist running a private practice in Baghdad, Iraq. He has experienced microscopic dentistry since 2017 in which he dealt with microscopes since 2017 and all the patients he treated were under the microscope, he has gained high skills and experience in using it. He is also an expert trainer specializing in anterior direct restorations and has given many courses about direct restorations in esthetic dentistry, in local and international conferences.

Course Title: Oral Medicine Fundamentals – Diagnosis and Management of Oral Mucosal Diseases

Abstract:

Dentists are experts in the diagnosis and management of diseases of the teeth and periodontium; however most have less confidence when it comes to “non-dental” oral diseases. Yet dentists are responsible for providing comprehensive oral health care and therefore must be familiar with the full spectrum of conditions that can affect the oral cavity. Following a foundational lecture, the audience will actively participate in a classroom style case-based review of oral medicine conditions, working through clinical descriptions, providing differential diagnoses, and proposing basic management considerations. Finally, through a live demonstration, attendees will learn surgical skills to perform a diagnostic oral soft tissue biopsy.

Introduction:

Dentists frequently encounter patients with oral mucosal lesions and can provide effective care for a wide spectrum of conditions.  Participants will learn a systematic approach to patient assessment including history taking, clinical examination, differential diagnosis, and the appropriate use of diagnostic tests. Clinical decision-making and principles of management will be reviewed, including use of topical and systemic therapies, in addition to non-pharmacologic measures. The interactive session will engage participants in the clinical reasoning process through a series of real-world oral medicine cases. Participants will actively participate in case discussions by responding to polling questions related to diagnosis, differential diagnosis, diagnostic investigations, and management decisions. The cases will highlight common and uncommon, but can’t miss oral conditions encountered in clinical practice, reinforcing the diagnostic framework introduced in the lecture while promoting audience engagement and collaborative learning.

Learning Objectives:

  • Be able to obtain a basic medical history and work-up for a patient with an oral mucosal lesion.
  • Recognize the clinical spectrum of diseases that can manifest in the oral cavity and be able to generate a rational differential diagnosis.
  • Understand the basic principles of safe and effective management of oral mucosal conditions.
  • Recognize when an oral mucosal biopsy is indicated and determine the optimal location and technique.
  • Appropriately refers a patient with an oral mucosal lesion to a specialist.

Live Demonstration:

  1. This module provides practical training in performing oral biopsy procedures. Following an informative lecture, the instructor will demonstrate basic biopsy techniques using synthetic tissue. The session will cover proper site selection, instrumentation, biopsy technique (incisional and excisional), suturing and use of silver nitrate sticks, specimen handling, and documentation. Emphasis will be placed on minimizing complications, ensuring diagnostic adequacy, and adhering to best practices in specimen preservation and submission for histopathologic and molecular evaluation.
  2. Procedure and Demonstration Steps:
    1. Review the necessary instruments and their proper use for performing an oral mucosal biopsy
    2. Provide adequate and appropriate local anaesthetic for performing and oral mucosal biopsy
    3. Select the optimal biopsy site and technique based on lesion characteristics and diagnostic needs
    4. Demonstrate proper technique for performing incisional and excisional oral biopsies using appropriate instruments and surgical principles
    5. Properly handle, label, and submit biopsy specimens to ensure accurate histopathologic diagnosis

About the Speakers:

Dr. Treister is an oral medicine specialist at Brigham and Women’s Hospital (BWH) and Clinical Director of Oral Medicine and Oral Oncology at the Dana-Farber Cancer Institute (DFCI) in Boston, Massachusetts, and Associate Professor of Oral Medicine at Harvard School of Dental Medicine. He is board certified in Oral Medicine and practices at BWH, DFCI, Boston Children's Hospital, and Dartmouth-Hitchcock Medical Center, with special interest in oral mucosal diseases, salivary gland diseases, orofacial pain disorders, and oral complications in cancer patients.

About the Speakers:

Prof. Andres Pinto DMD, MBA, MSCE
Dr. Pinto is founding Chair and Professor of Oral and Maxillofacial Medicine in the Department of Oral and Maxillofacial Medicine and Diagnostic Sciences at Case Western Reserve University, School of Dental Medicine in Cleveland, Ohio. In addition, he is an attending staff in the Department of Surgery, Divisions of Oral and Maxillofacial Surgery and Pediatric Dentistry at University Hospitals Case Medical Center and Rainbow Babies and Children’s Hospital, and the director of the Oral Medicine Fellowship program and the MScD in Oral Medicine graduate program. He received his DMD, MPH, MSCE, certificate in Clinical Research, and certificate in Oral Medicine, from the University of Pennsylvania, School of Dental Medicine and School of Medicine. Dr. Pinto is a diplomate of the American Board of Oral Medicine and the European Association of Oral Medicine and holds a FDS from the Royal College of Surgeons (Edinburgh). He was a member of the faculty and director of Oral Medicine Services at the University of Pennsylvania for 13 years, and during 2011–2013 he was the Chief of the Division of Community Oral Health at Penn. He has published extensively and is a coauthor of several major texts in Oral Medicine, like Burket’s Oral Medicine, Dental Clinical Advisor, and Curbside Consultation in Pediatrics. He is co-editor of Risk Assessment and Oral Diagnostics in Clinical Dentistry, a popular reference book for dental students and residents in General Practice and Oral Medicine training. He maintains a busy referral practice in Oral Medicine and Orofacial Pain at UH Case Medical Center. His clinical interests are the management of oral mucosal disease, oral oncology, pediatric oral medicine, and orofacial pain. Dr. Nikolaos Nikitakis, MD, DDS, PhD
Professor Nikolaos G. Nikitakis received his DDS and MD from the University of Athens, Greece and his Certificate and PhD in Oral and Maxillofacial Pathology from the University of Maryland, Baltimore, USA. He is a Diplomate of the American Board of Oral Maxillofacial Pathology and a Fellow of the American Academy of Oral and Maxillofacial Pathology (AAOMP) and the American Academy of Oral Medicine (AAOM).
He has served as a Director of the PhD and Residency Program in Oral and Maxillofacial Pathology at the University of Maryland. Currently, he is the Chair of the Department of Oral Medicine & Pathology and Hospital Dentistry, School of Dentistry, National and Kapodistrian University of Athens, Greece
His clinical interests include clinical and histopathologic diagnosis and management of oral and maxillofacial diseases. His research interests primarily focus on oral, head and neck oncology / cancer research. He has received several honors and awards, including the Waldron Award (AAOMP) and the Hamilton B.G. Robinson Prize (OOOO). He has over 180 scientific publications with more than 2,000 citations (h index: 27). He also serves as an Editorial Board member of OOOO (Oral and Maxillofacial Pathology Section) and Oral Diseases, and as a reviewer of various scientific journals. He is the past President of the European Association of Oral Medicine (EAOM) and the Hellenic Society of Oral Medicine and Pathology (HSOMP).

Dr. Omar Kujan, BDS DipOPath, MDSc, MFDS RCSP (Glas), PhD FHEA FRCPath (UK), FFOMP RCPA (Australasia)
Associate Professor Omar Kujan received his PhD from the University of Manchester, School of Dentistry, in 2006. The University of Manchester appointed him as a Clinical Research Fellow to develop and assess an innovative oral cytobrush. Afterwards, he took a training post at the Oral Pathology Department, Guy's Hospital, King's College of London, United Kingdom. He most recently joined the Oral Health Centre and School of Dentistry at the University of Western Australia in 2016 as a lead discipline in oral pathology. He is an experienced and AHPRA registered oral and maxillofacial pathology specialist and received the fellowships of the Royal College of Pathologists from the UK and Australasia. Omar contributed significantly to developing the 2020 updated classification of oral potentially malignant disorders by the WHO Collaborating Centre for Oral Cancer. The WHO Head and Neck Tumours classification has adopted his proposed binary oral epithelial dysplasia grading system. Dr Kujan leads the Oral and Maxillofacial Biology and Diseases Research Stream at UWA Dental School. He has achieved a remarkable track record, demonstrated by his successful research grant applications and publication of high-impact research papers. He currently supervises 8 PhD students and several DCD and DMD students. He is an associate editor of the Journal of Oral Pathology and Medicine, Frontiers in Oncology, Scientific Reports (Nature) and Frontiers in Oral Health (Oral Cancer Section). He regularly reviews leading journals in dentistry, cancer and oral diseases.

Dr. Hend Alqaderi – USA, BDS, DMSc
Dr. Hend Alqaderi is an academic and researcher specializing in Dental Public Health and Pediatric Dentistry. She holds a BDS and DMSc, reflecting advanced training in clinical dentistry and population-based oral health research. She serves as an Assistant Professor in the Department of Public Health and Community Service at the Tufts University School of Dental Medicine, where she is involved in teaching, research, and community-oriented oral health programs. Her research focuses on dental caries prevention, oral health disparities, and improving access to dental care for children and underserved populations. She has published in peer-reviewed journals and actively contributes to academic collaborations in preventive dentistry and public health. Dr. Alqaderi is dedicated to advancing oral health through education, research, and community engagement, while mentoring future dental professionals in evidence-based preventive care.
